Manager, Care Coordination

Job Description
Long Term Care Partners is currently seeking a seasoned Manager to provide leadership and direction to a group of approximately 15 (and growing) RN Care Coordinators who telephonically manage the care of Federal Family members who are ready to go into claim (or are already in claim status) for Long Term Care services across the country.

Care Coordination encompasses benefit eligibility determinations, customer relations, benefit authorizations, and referrals. In addition to the people management aspect of the position, the incumbent will be responsible for managing the Care Coordination process, reviewing and revising policies and procedures, and overseeing the Quality Control process for the Care Coordination team. Strict service levels are associated with our program, so incumbent must be experienced and comfortable with data gathering and reporting.
Job Requirements
Requirements include:
  • Registered Nurse, either currently licensed in the state of NH or qualified to be licensed in the state of NH
  • 3+ years experience as either a Supervisor or Manager in a Case Management/Care Coordination department in one of the following industries: health insurance, disability insurance, Health Care Organization, or Long Term Care insurance
  • Detailed knowledge of healthcare delivery system, community services and state/private health plans claims guidelines, processes and procedures
  • Proven ability to effectively manage staff and provide effective coaching/development
  • Self-motivated with strongly developed organizational skills, plus ability to manage multiple priorities
  • Effective oral and written communication
